Twenty reagents and 51 potions. Every mix the game accepts is listed — 57 recipes in all — so if a pair is not here, it does nothing.

How mixing works

A reagent dropped into an empty bottle gives a base potion of that reagent's colour, and the reagent's potency sets its strength. Two potions mixed together give something further up the tree. Mixing a pair the game does not recognise wastes both and leaves you a base potion, so the pairs below are the whole of it.

Strength is the reagent's potency plus your Alchemy skill, so the same recipe brewed by a better alchemist is a better potion — which is why the expensive reagents matter for anything you intend to keep. Rank also gates what you can attempt at all: Normal mixes base potions into secondary ones, Expert reaches the compounds, Master the whites and Grandmaster the blacks. Bottles and reagents are sold at any alchemist, and the Alchemy skill governs what you can brew without ruining it.

The base potions are left without a recipe list on purpose: a failed mix returns one of them, so the table pairs dozens of combinations with each that are not recipes so much as the game telling you the mix was wasted. Data status.
